Completed application packets consist of:

• Internship Application Form
• Official Transcripts
• Verification Statement of Completion of Dietetic Program
- or -
• Declaration of Intent to Complete Dietetic Program
• Letter of Application (typed) - 2 page maximum
• Three sealed Letters of Recommendation
• Application Fee of $75.00 payable to Olive View Internship. Non-refundable.

Mail the completed application to the Dietetic Internship Director by the date specified on the deadlines page.
